Families participate in our case management services from the first day they arrive at PEC. Working together with a case worker, clients create a unique action plan to obtain and maintain affordable housing by achieving short- and long-term goals. The ratio of case managers to emergency and transitional housing families is 16:1, which ensures substantive, individual attention.
The case managers utilize a strengths-based and trauma-informed approach to engage families in the development of their service plan and provide weekly coaching or counseling to facilitate goal attainment.
Using a comprehensive catalog of assesssments and the family service plan as the guide, case managers link family members to appropriate resources and service providers within the community. Case managers also monitor the utilization and quality of the solutions; offer solutions for eliminating service delivery obstacles; and make recommendations for additional linkages with additional service providers as needed.
